Before attending the vaccination clinic, we ask that you provide some basic information about your dog’s health and behaviour.

This information helps our veterinary team prepare for your dog’s visit and helps us provide a safe and comfortable environment for dogs, owners, volunteers and veterinary staff.

Please answer all questions honestly. If your dog has behavioural or handling needs, letting us know in advance allows us to make appropriate arrangements.
